Responsibilities: 

An non-exempt position, responsible for New Patient Welcome Visits to ensure that the patient medical record is updated to reflect current medications and health conditions and that any immediate care needs or referral requests have been addressed prior to the New Patient Provider appointment while working collaboratively with Primary Care Providers and Clinical Teams.

  • Review and confirm medication dosages are reconciled and accurately listed in patient’s medical record.
  • Discuss TVH medication refill policy and TVH use of payer automatic delivery services. Provide materials for patient to access payer portals.
  • Educate process for acute care visit needs prior to New Patient Provider Visit.
  • Refill maintenance medications as needed to get patient through to scheduled Provider appointment.
  • Review New Patient Packet information and verify health histories ensuring all is documented in the medical record.
  • Requests previous medical records.
  • Educate New Patient on clinical aspects of care delivery and model of care at TVH.
  • Offer COVID and/or Influenza Vaccinations.
  • Collaborates and communicates with PCP, Care Teams and NPS within patient assigned Care Center to ensure patient’s needs are met and experiences a successful onboarding experience.
  • Other duties as assigned.

Education/Experience Requirements:

  • Current, unrestricted RN or LPN license in the state of Florida.
  • Case Management experience
  • 5+ years’ experience in a hospital or acute care setting with direct patient care
  • Knowledge of nursing principles, practices and procedures; budgetary, supervisory, human resources and quality improvement principles and techniques; clinical state of the art advancements and trends in health care, particularly in the area of Patient Centered Medical Home.
  • Knowledge of medical terminology, anatomy, physiology, and pathophysiology.
  • Familiarity with health care systems, electronic medical records, regulations, policies, and functions.
  • Understanding of documentation standards.
  • Knowledge of equipment, supplies, and material needed for medical treatment.
  • Understanding of basic laboratory, procedures including preparation and screening.
  • Knowledge of infectious disease management and control and safety standards.
  • Skill in applying and modifying, as appropriate, the principles, methods and techniques of professional nursing
  • Skill in identifying problems, researching, and recommending solutions
  • Skill in developing and maintaining high levels of quality care and quality assurance
  • Skill in exercising a high degree of initiative, judgment, discretion, and decision-making.
  • Coaching ability.
  • Excellent clinical skills and judgment.
  • Excellent written and verbal communication skills, with the ability to build relationships.
  • Excellent organizational skills
  • Strong ability to work autonomously and be self-directed.

Salary is commensurate with experience.
