Responsibilities: 
A nonexempt position responsible to support the Quality Assurance/Quality Improvement initiatives at The Villages Health. This position will coordinate quality process, educate and train Care Center Providers and staff regarding quality initiatives and process improvement and assess and report data related, but not limited to PCMH, MIPS and HEDIS Measures. The coordinator will be responsible, under the direction and guidance of the Utilization and Compliance Manager, for directing, assisting and guiding management and staff toward capture of all Quality Measures by conducting ongoing monitoring of data systems, identifying and analyzing deficiencies and assisting with completion of corrective actions.
  • Review and audit MIPS data within our EMR to ensure capture and scores maintained or improved. Communicate and educate Care Gaps to appropriate Providers/Teams and Care Center Leadership.
  • Review and audit HEDIS Measures data from payers to ensure capture and compliance within each for all Care Centers. Communicate and educate Care Gaps to appropriate Providers/Teams and Care Center Leadership and facilitate closure of Care Gaps.
  • Assist with developing annual PCMH submission data, based on ongoing Performance Improvement Initiatives throughout the year and audit initiatives, compile information and data for each Care Center and submit annually.
  • Coordinates and facilitates meetings with Care Center Teams to review progress in quality programs and leads process improvement teams and initiative.
  • Educates staff on proper documentation and collection of quality data information within the EMR.
  • Work with payer representatives to accomplish goals and care gap closures.
  • Builds rapport and establishes trusted relationships with Providers and Care Teams.
  • Acts as a resource for questions related to quality initiatives, process improvement and care gap closure needs.
  • Provides excellent customer service to Providers, Care Teams, Care Center Leadership, colleagues and external partners.
  • Maintains excellent knowledge of MIPS, HEDIS and PCMH Quality Standards and process.
  • Identifies and recommends opportunities for improvement.
  • Retrieves and inputs data and documentation into Payor Portals in order to close Care Gaps.
  • Other duties as assigned.
Education/Experience Requirements:
  • 2 plus years of customer service experience analyzing and solving problems OR 1 year of experience in an office setting environment using the telephone and computer as the primary instrument to perform job duties
  • High school education or GED, RMA or CMA Certification preferred.
  • Extensive knowledge of medical terminology preferred.
  • Experience with computer databases to track and analyze data.
  • Experience with HEDIS, MIPS and PCMH, a plus.
Communication and Work Skills
  • Excellent organizational, interpersonal, presentation, written and verbal communication skills.
  • Strong communication, writing and presentation skills and must be comfortable presenting to healthcare staff and Leadership.
  • Keen attention to detail and ability to prioritize and handle multiple assignments.
  • Ability to communicate in person, by phone, or via e-mail in a professional and friendly manner.
  • Excellent customer service skills and attention to detail.
  • Ability to empathize, motivate and encourage people toward change, regardless of culture, religion or economic background
  • Ability to work and interact with professionals as part of an interdisciplinary team
  • Self-motivated, flexible individual who can flourish in an office setting with colleagues as well as in a self structured independent setting

  • Ability to multi-task, this includes ability to understand multiple services and multiple delivery modes within each service
  • Ability to prioritize and organize own work to meet agreed upon deadlines.
  • Ability to travel and present at all Care Centers throughout TVH.
Technology Skills
  • Ability to effectively use multiple technology systems to assist with identification and enrollment of patients and review patient data
  • Strong PC computing experience (Microsoft Office and web based tools)
  • Familiarity with computer and Windows PC applications, which includes the ability to learn new and complex computer system applications and apply their use
